Iatrogenic Meatal Obstruction After Circumcision Leading to Acute Renal Failure
A 5-year-old boy, underwent ritual circumcision performed by a general Physician. The immediate postoperative course was marked by progressive dysuria, which rapidly evolved into complete anuria within the first 24 hours. The child was brought to the emergency department with severe lower abdominal discomfort, restlessness, and progressive abdominal distension. On physical examination, he was irritable, mildly dehydrated, and a large, tense suprapubic mass was palpable, consistent with a markedly distended bladder (Figure N°1).
Initial attempts at urethral catheterization in the referring facility had failed, which was the primary reason for referral to our pediatric surgery department. On admission, the child’s vital signs were stable, but clinical and laboratory evaluation revealed acute renal impairment, with elevated blood urea nitrogen and serum creatinine levels, along with oliguria evolving to complete anuria.
